Registration Instructions<\/h2>\n
Please verify your membership status before registering<\/strong>. If you need to renew or would like to join, then proceed with the membership first then register for the meeting. Or add the membership to your cart BEFORE registering for the meeting. You may also renew or join with the membership form.<\/p>\nIf you are submitting a registration on behalf of another<\/strong>, please fill out the registration with the attendee\u2019s information. DO NOT login using your information. You MUST be logged in under the attendee. Name, email address, phone number, mailing address and badge info should be the attendee\u2019s information and not yours. Your name should not appear anywhere on the registration. The only exception is the name on the credit card.<\/p>\nIf you are submitting registrations for multiple people<\/strong>, it is highly recommended that you use the registration form and email, mail or fax them in. If you choose to register them online then please follow the instructions above.<\/p>\n\n- Do no submit multiple registrations logged in under the same account.<\/li>\n
- After completing the first, you MUST LOG OUT before submitting the next registration. Each attendee needs their own account. Failure to do so will submit inaccurate information.<\/li>\n<\/ul>\n
If you need a confirmation\/receipt of the registration<\/strong>, you have the option of providing an additional email address on the \u201cCheckout\u201d screen.<\/p>\nCancellations<\/h2>\n
Cancellation requests for refunds must be submitted in writing to registrar@simbhq.org before 5 pm eastern daylight savings time, October 4, 2018<\/strong>. Refunds will be issued, less a $100.00 USD administrative fee. No refunds will be issued after October 4, 2018<\/strong>.<\/p>\nRegistration List Policy<\/h2>\n
By registering for this meeting, you agree that SIMB may maintain your name and employer information completed in this form as necessary for its activities including that SIMB may publish the information in a list of registrants available to meeting attendees. Reproduction of the list of registrants is not authorized and its use for general mailings or similar purpose, including use as a general mailing list for invitation to functions at SIMB meetings not specifically authorized by SIMB is prohibited.<\/p>\n<\/div>

Wire Transfer Payments<\/h2>\n
The wire transfer option is provided only for international attendees who are not able to pay by other means. A wire transfer fee of $40<\/strong> will be added to your registration total. Payment must be in U.S. Dollars<\/strong>.<\/p>\n
